Why Representation Matters in Healthcare Research

Healthcare research shapes the medicines we use, the treatments we receive, and the health policies that govern our communities. Yet for decades, many of these studies have been conducted on populations that do not reflect the diversity of the people they are meant to help. For African and First Nations communities in Australia, this gap has real and lasting consequences.

1. The Research Gap and Why It Exists

Clinical trials and medical research have historically overrepresented certain population groups — particularly white, middle-aged men from high-income countries. This was not always intentional, but the result has been a body of medical knowledge that may not apply equally across different ethnicities, cultures, and socioeconomic backgrounds.

For Aboriginal and Torres Strait Islander peoples and African Australian communities, this gap means that treatments, dosages, and screening guidelines may have been developed without their specific health needs, genetic profiles, or social determinants in mind.

1.1  Genetic and Biological Differences

Different population groups can respond differently to medications and treatments due to genetic variations. For example, certain drug metabolisms differ significantly across ethnic groups. When research is conducted without diverse representation, these differences go undetected, leading to treatments that may be less effective or carry greater risks for underrepresented populations.

1.2  Social Determinants of Health

Health is not determined by biology alone. Where people live, their access to food and clean water, their employment conditions, education levels, and exposure to historical trauma all influence health outcomes. Research that does not account for these social determinants produces findings that are incomplete at best and misleading at worst for communities facing systemic disadvantage.

2. The Impact on Aboriginal and African Australian Communities

In Australia, Aboriginal and Torres Strait Islander peoples experience significantly higher rates of chronic disease, cancer, and mental health challenges compared to the broader population. African Australians, many of whom arrived as refugees, face additional barriers including language, cultural differences, and mistrust of health systems shaped by previous harmful experiences.

When medical research does not include these communities, the resulting guidelines and health programs often fail to address their actual needs. Cancer screening programs, for instance, may not be designed with cultural safety in mind, leading to lower participation rates and later diagnoses.

2.1  Cancer Research and Underrepresentation

Studies on cancer risk factors, treatment efficacy, and survival rates have largely been conducted on Caucasian populations. This creates a significant blind spot when applying findings to Aboriginal Australians, who have higher mortality rates from several cancers including lung, bowel, and cervical cancer, or to African Australians who may carry different genetic risk profiles for conditions such as prostate cancer.

2.2  Mental Health Research

Mental health research has also failed to adequately represent culturally diverse populations. The experience of grief, trauma, and psychological distress within First Nations communities — deeply connected to culture, country, and community — is fundamentally different from what is captured in mainstream psychiatric literature. Similarly, the mental health needs of African refugees who have experienced war, displacement, and loss require culturally grounded approaches that most research has not yet developed.

3. What Meaningful Representation Looks Like

True representation in healthcare research goes beyond simply recruiting more diverse participants. It requires communities to be involved at every stage of the research process — from defining the research questions to interpreting findings and translating results into community-led action.

3.1  Community-Controlled Research

Aboriginal community controlled health organisations have long advocated for research that is led by and for their communities. This model ensures that research questions reflect genuine community priorities, that data collection is culturally safe, and that findings are returned to communities in ways that are meaningful and actionable.

3.2  Cultural Safety in Research Settings

Researchers must be trained in cultural safety and work alongside community health workers and elders. Consent processes must be conducted in community languages, and communities must have the right to withdraw their data or decline participation without consequence.

3.3  Inclusive Clinical Trials

Healthcare institutions and pharmaceutical companies must actively recruit participants from underrepresented communities, remove barriers to participation such as transport and language, and design studies that are relevant to diverse populations. Financial incentives for participation and flexible scheduling can help make inclusion practical rather than aspirational.

4. The Role of Digital Health in Closing the Gap

Digital health platforms like Precision Digital Health are working to address representation gaps by collecting data from diverse communities, developing culturally appropriate health resources, and supporting research that centres the voices and experiences of Aboriginal and African Australian people.

When communities have access to platforms that speak their language, understand their culture, and respect their sovereignty over their own health data, the quality and relevance of health research improves for everyone. Representation is not just a matter of fairness — it is a matter of scientific accuracy and community survival.
